JOB ANNOUNCEMENT – Patient Navigator

Citrine Health is a non-profit organization that connects women and their families to health care, social service, and disease prevention programs.

Breast, Cervical and Colon Health Program (BCCHP) is administered by the Department of Health (DOH) through a grant from the Centers of Disease Control and Prevention (CDC). The objective of BCCHP is to enroll eligible people for breast, cervical and colon cancer screening services at no cost to clients.

Patient Navigation is individualized assistance offered to clients to help overcome health care system barriers and facilitate timely access to quality screenings and diagnostics, as well as initiation of treatment services for women diagnosed with cancer. Patient Navigation at Citrine Health is a crucial position for the six counties that we serve: Island, Pierce, San Juan, Skagit, Snohomish, and Whatcom.

Specific responsibilities will include (but are not limited to):
  • Implement a Patient Navigation program that follows a researched-based protocol.
  • Write assessments of the clients’ barriers to cancer screening, diagnostic services, and if necessary, initiate cancer treatment.
  • Provide Client education and support.
  • Resolve client barriers (for example, transportation and translation services).
  • Track Client and monitor progress in completing screening, diagnostic testing, and initiating cancer treatment.
  • Collect data to evaluate the primary outcomes of patient navigation that includes client adherence to cancer screening, diagnostic testing, and treatment initiation.
  • Enter and maintain essential spreadsheets and database records.
  • Liaison between client, provider and Health Care Authority (HCA) in maintaining enrollment in Medicaid for treatment
  • Attend meetings as designated by Citrine Health Management
  • Other duties as assigned.

Finally, the ideal candidate should also have an understanding of social work and/or health practices.

Specific requirements include:
  • Prefer MSW, or any combination of work experience & education to perform duties.
  • Experience working with database programs and records.
  • Ability to work both independently and with groups,
  • Demonstrate strong verbal and written communications skills (bilingual Spanish a plus),
  • Excellent organizational skills,
  • Demonstrate strong computer and Internet skills (QuickBooks, Microsoft Word, Excel, Publisher and Outlook).

  • Location: 2940 W Marine View Dr, Everett, Washington
  • Reports to: WA Breast, Cervical and Colon Health Director
  • Hours/FTE: 40 hours with health insurance, dental, vacation, sick and includes membership to Women’s Wellness Center
  • Wage: $40,000 - $73,500 DOE
  • Travel: Some Regional Travel; Valid WA Driver's License; Reliable Transportation
